If Teemo stands still and takes no actions for a short duration, he becomes Invisible indefinitely. If he's in brush, Teemo can enter and maintain his Invisibility while moving. After leaving Invisibility, Teemo gains the Element of Surprise, increasing his Attack Speed for a few seconds.

How to counter and exploit weaknesses
Buy Oracle Lens fast, it reveals and disables Teemo's mushrooms.
Q (Blind) has an 8s cooldown at rank 1: engage right after he uses it if you're an auto-attack champion.
Teemo has no dash and no hard CC — ganks and forced engages are very effective against him.
In the late game, buy Control Wards to clear mushrooms before contesting Baron or Dragon.
