Tips to maximize your impact

Prepare your graves
Hit a champion or last-hit a minion with Last Rites (Q) to dig a grave: with three graves nearby, recast the spell to raise Mist Walkers from them.
Mark your target with your Mist
Hit your target with Mourning Mist (E): your nearby graves keep raising Mist Walkers, the target loses armor, and your units rush toward them.
Wall in your target
Trap your target in Dark Procession (W): the wall blocks enemies but not your allies, and your Mist Walkers hit them in the meantime.
Split push with your Maiden
After 10 seconds, recast Eulogy of the Isles (R) to send the Maiden of the Mist to push the nearest lane, but you lose a big part of your power.

How to counter and exploit weaknesses

Dodge his Mourning Mist
Dodge Mourning Mist (E): once marked, you lose armor, his nearby graves keep raising Mist Walkers, and his units rush toward you.
Kill his Mist Walkers first
Kill his Mist Walkers, summoned by Shepherd of Souls (passive), before going for him: a single auto or single-target spell is enough for each.
Break his wall
If Dark Procession (W) traps you, hit the wall to break it instead of waiting for it to disappear.
Kill the Maiden first
If you can, take down the Maiden of the Mist before Yorick: summoned by Eulogy of the Isles (R), she carries a large part of his damage and push.
Answer his split push
Don't let him split push alone: with the Eulogy of the Isles (R) Maiden and his Mist Walkers, he quickly threatens a tower. Send someone to answer him.
